Circle of Life Hospice House is a Wisconsin DHS-licensed, four-bed Adult Family Home dedicated exclusively to hospice-level care. We provide a peaceful, home-like environment where individuals can receive compassionate, dignified care at the end of life—surrounded by comfort, respect, and support.

We specialize in 24-hour hospice care, working in close collaboration with licensed hospice agencies to ensure each resident’s medical, emotional, and spiritual needs are met with excellence and consistency.

Our Vision
Admission Criteria
To ensure the highest level of care and safety for all residents, the following criteria apply:

The individual must be enrolled with a licensed hospice agency
​

Care needs must meet Wisconsin DHS Chapter 88 requirements, as assessed by Circle of Life administrative staff prior to admission
​

The resident must not pose a danger to themselves or others
​

The facility must be able to safely and appropriately meet the resident’s care needs
​

Each admission decision is made thoughtfully, with the well-being of the resident, their family, and our care environment as the highest priority.
