Circle of Life Residence

To ensure the highest level of care and safety for all residents, the following criteria apply:

 

• The individual must be enrolled with a licensed hospice agency

​

• Care needs must meet Wisconsin DHS Chapter 88 requirements, as assessed by Circle of Life administrative staff prior to admission

​

• The resident must not pose a danger to themselves or others

​

• The facility must be able to safely and appropriately meet the resident’s care needs

​

• Each admission decision is made thoughtfully, with the well-being of the resident, their family, and our care environment as the highest priority.
